Games : Why Does My Minecraft Crash During Multiplayer When Everyone else is ok

Whenever I either die in mincraft, login, or teleport somewhere, my minecraft crashes with the error below. The only server that this happens to consistently with is CosmicPvP( The IP is play.cosmicpvp.me). On other severs this happens sometimes but not consistently.

To try and fix it, I have uninstalled and reinstalled java, I have re- downloaded minecraft on minecraft.net (this works most of the time but after I either log out, teleport, or die it crashes again and I can't get back on.
