Games : Sky Factory 2 permanent blindness and poison effect

I went AFK while playing Sky Factory 2. When I returned, my character was afflicted by the blindness and poison effects. I've tried drinking Milk to clear them, and I've also tried the /effect username clear command but neither worked. How do I get rid of these effects?
